Toolbars disappear

It has been normal for a while but all the sudden new windows come up with Separate Location Bar only. Can be fixed in View/Preferences but after a while it happens again.

Yes. Select View > Toolbars > Navigation Bar. You'll probably need to re-add the address bar to it which can be done via the "Customize Navigation Bar" menu item also found in the Toolbars menu above.
