Sorry for the trouble! Do you mean "totally out of sync" as "are not syncing with each other", or do you mean "they are syncing, but I'm not seeing the same things on each device"?

If it's the latter, the first thing I'd try is opening up Project view on the iOS device, then tapping the View button (looks like an eye) and setting the filter to "All".

At the same time, on the Mac, open the Perspectives menu and select the "All Items" perspective. Those two views should show just about everything in the database, helping you determine if the devices are actually out of sync, or if the filter settings on the two devices were just showing different things...
