Off the top of my head, it sounds like you're right on the edge of whatever radius is assigned for the context(s) in question. As iOS periodically checks your location, sometimes the results indicate that you're inside the radius and other times they say you aren't.

If the apparent location of the device bounces between those states, it could cause iOS to trigger those reminders more than once...
