I also struggle with the concept of "Defer" as "Start", but as I started thinking about it in a GTD perspective, it made a bit more sense. David says that you should not use due dates, using the calendar for hard date items. If you have only a start date, and no due date, then "Defer" actually does work as I would expect as it pushes something out into the future.

The challenge is that my reality and David's view are not always aligned, and I indeed have due dates for probably more than I should from a GTD purist point of view.
