For some reason in quick entry or basically just adding in a new action, my context menu has duplicates but when it go to the context pane there is only one of each context.

In my context dropdowns I see:

mac
textmate
errands
home
mac
textmate
errands
home

Why do I have duplicates and how do I remove them without orphaning my actions?
