version 95000 does not seem to print checkboxes. On version 94950 the checkboxes were printing.
This has been reported.

I have the same problem. The only way that I can get a list is from context view, I can't get one from planning view.

This should be fixed in the most recent builds -- in earlier builds you had to go to Page Setup and hit OK at least once to confirm the settings, but as of recent builds this should work out of the box.
