All the settings look to be OK. OF is activated, and all the different actions are set to display a notification.

The latest Growl release no longer supports alerts from the older Growl SDK which is currently being used in OmniFocus. We're integrating the latest Growl SDK into OmniFocus, which should solve this problem.
