I know it's an obvious question but do you have a third party hack that uses keystrokes? It could be a dedicated itunes front end (coversutra, synergy etc) or triggers in Quickeys, Quicksilver or Launchbar

Not that I know of.
I do have Quicksilver installed but never use it..
I've also got Butler installed but have never gave it special keys for itunes.

I'm not sure if this was the default or user-defined, but I have a global hotkey set through quicksilver to turn on iTunes as ctrl-option-space.

See if this is your problem - click on quicksilver menu item - go to triggers - select itunes on the left hand pane - and see if you have any checked off.
