I created an outline a few days ago and picked a theme that I've been liking, but I can no longer remember which one it is. Is there somewhere where it will tell me the name of the current template or theme? If there is, I haven't found it ...

No, there's no way to do that aside from matching up the visual appearance. There's no link to the theme after applying it or after creating a new file from a template. If you want a bigger preview of the themes/templates, you can push the space bar to open the Quick Look preview. If you still can't find it, you can always save your current document as a template.

Likely an issue with Launch Services. Do you have a copy of OmniOutliner 3 on your Mac? If so, it might be trying to use the Quick Look generator in that app which doesn't work on 10.9. Deleting OO3 should fix it, or if you don't want to do that or that isn't the issue, please contact support and we'll help you figure out how to fix this. Thanks!
