The only items without a context are the unprocessed items in my Inbox. The only time I move something out of the Inbox is by assigning it a context and making it a project or assigning it to another project.

The only projects on hold are the few that I use as templates or the many ideas that I keep in a Someday folder. The template projects include start and end dates, too. Could these items be the ones that are indicated by not appearing? It doesn't seem likely because together there are dozens of items in these projects on hold and I only notice this discrepancy with a few items.
 
Hard for me to say what is happening to you. I just wanted to extend the list of possible reasons why you might see such behavior even if the program is working as designed. Of course, there's always the possibility that it isn't :)
