Unfortunately, this is not possible in OF for iPhone at this point since it's a limitation of the iPhone SDK -- apps cannot run in the background
In fact, there are no official iPhone applications that can support alarms or reminders on the iPhone -- at least not unless they're actually running (which usually defeats the purpose).

OmniFocus on the iPhone can't sync directly to the calendar on the iPhone.
However, you could sync to OmniFocus on your Mac, then use iCal syncing to sync to iCal on your Mac. Then that could sync to your iPhone through iTunes or MobileMe. But even then you'd have to go back and add alarms to the items manually in iCal on either the phone or desktop.

Except that OmniFocus creates tasks in iCal rather than events, and these tasks won't sync to the iPhone as the iPhone does not support tasks.
Or am I missing some hidden feature in OF that can sync OF items into actual calendar events?
